TEAM 2 – Migraine
Migraine is listed as the sixth most disabling disorder globally and as the most disabling of all neurological disorders by the World Health Organization, with important societal costs (Global, regional, and national burden of neurological disorders, 1990–2016: a systematic analysis for the Global Burden of Disease Study 2016, Lancet Neurol 2019). Scientific evidence supports that migraine pathophysiology involves (i) inherited alteration of brain excitability, (ii) intracranial arterial dilatation, associated with plasma extravasation and mast cell degranulation in relation to a state of ‘sterile’ or neurogenic inflammation of intracranial meninges, in response to the release of neuropeptides such as substance P or calcitonin gene related peptide (CGRP) from peripheral trigeminal innervation (Goadsby et al. Physiol Rev 2017); (iii) recurrent activation and sensitization of the trigemino-cervical complex (TCC) (Burstein et al. J Neurophysiol 1998; Boyer et al. Pain 2014). Clinically, migraine is characterized by attacks of unilateral, throbbing head pain, with a clear female predominance, which are associated with sensitivity to movements and to visual, auditory and other afferent inputs – i.e. general sensitization including allodynia (Rasmussen et al. Cephalalgia 1992). Prodromes, such as tiredness, irritability and yawning, can precede headache by up to 48 h (Giffin et al. Neurology 2003). Postdromes, including feeling unwell and tiredness, follow most attacks for up to 24 hours (Giffin et al. Neurology 2016.). On top of that, some migraineurs can develop chronic migraine, characterized by headaches for more than 15 days a month (May and Schulte. Nat Rev Neurol 2016), and which accounts for the majority of the clinical, financial, and emotional burden of migraine (May and Schulte. Nat Rev Neurol 2016). In other words, migraine is more than headache: it can be considered as an inflammatory disease associated with a paroxysmal alteration in the gain, or input-output modulation (Haider et al. Neuron 2009), of multiple sensory systems. Our project will tackle these 2 aspects of migraine at both the preclinical and clinical levels.
